Please print and complete the application below and return it to the parish office, located at La Asuncion Church, 777 Cortlandt St. 

Once received the pastor will schedule an appointment to speak with you to secure the date.

1. Couple must be registered in the church
2. The wedding should be take place in a year
3. Couple must have their papers in order, for example:

• Certificate of Baptism (updated)
• Certificate of  Confirmation (update)
• Letter of annulment (if was married before)

Una vez recibida, el párroco programará una cita para hablar con usted y asegurar la fecha.

1. La pareja debe estar registrada en la iglesia
2. La boda debe tener lugar en un año
3. La pareja debe tener sus papeles en regla, por ejemplo:

• Certificado de Bautismo (actualizado)
• Certificado de Confirmación (actualizada)
• Carta de anulación (si estuvo casado antes)
